I have been on dialisis 7 years and 5 years out on a transplant. My bones and muscles her bad, what can I do ?

I was on dialysis for 7 years and now am 5 years out on a transplant. My muscles and joints are in terrible pain, walking is getting difficult. Why and what can I do?
